A clinical study of 80 cases of choleithiasis

Abstract
80 consecutive cases of cholelithiasis treated surgically at St. Joseph Hospital from June 1964 to August 1970 were studied.
There were several differences in their manifestation of oriental population contrast to report in the western literature, that is, as is common to many races of oriental countries, bile duct stones are more common in Korean than European. Author believed that these differences should be properly reflected in the treatment of these people by modifying certain therapeutic principles.
The following results were obtained.
1) The highest incidence of cholelithiasis was found in the 5th decade and the sex ratio was 1 male to 15 female.
2) The majority of the patients were operated within 6 months after the onset of the disease.
3) The most prominent symptoms and signs were pain(92.5%) in the R.U.Q., epigastric region, etc.
4) In laboratory data, leucocytosis was noted in 55%, and in the cases combined with suppurative lesions, such as liver abscess or empyema of gallbladder the leucocytosis was much more increased.
5) On the X-ray of simple abdomen of 41 cases, gallstones were visualized or strongly suggestive in10 cases. On cholangiographic Analysis in 30 cases, 25 cases were not visualized in the gallbladder and 5 cases showed normal function of gallbladder with good visualization of the gallbladder.
6) In the location of cholelithiasis of 80 cases, 20 cases showed stones in the gallbladder alone, 39 in the common bile duct, 12 in the both side and 9 in the hepatic duct.
7) In the gross appearance of the stones, 18 cases were made of bilirubin, 10 cases cholesterol and remaindee 26 cases mixed type.
8) The majority of cholelithiasis, 81.25% of this series, was performed the cholecystectomy with choledochostomy and the additional procedure.
9) Postoperative complications were 14 cases, in which the wound infection was most frequent.
10) The mortality rate of this series was 2.5%.
